Move some build docs around for clarity. Move the TARGET_DEBUG flags from the end of T.cc to the beginning so that -Ox flags can be passed to make for specific binaries without having to reconfigure.

Makefile.linux-generic

Split the core-most CFLAGS, which should not be overridable (like -fPIC), into CFLAGS.core, so that went CFLAGS=... is passed on in a make invocation the -fPIC flag is not lost (as reported in [forum:39f8c54391d38c72 | forum post 39f8c543]).

Performance optimization in OP_Column: decode serial types 0 through 6, 8, and 9 (NULL and all integer widths) inline in a switch, avoiding the call and dispatch overhead of sqlite3VdbeSerialGet(). The wide-integer cases 5 and 6 add bulk, but omitting them would leave those types paying the switch miss plus the original call. Other serial types are decoded as before.
